Just as a note, that it is an awful constructed survey with a massive inbuilt bias due to the nature of the questions. The first part should ask for height/weight to put into context if someone is underweight/overweight. The second part is very random after answering the first part of the survey.

There's a double negative at question 26. Probably too late to change now I'm guessing but maybe worth pointing out that people might read it two different ways.
