I think the Akpom and Dijksteel ones were the same except the Rotherham player was behind akpom when he clipped him.
Yup for Akpom, especially when McGree, about five minutes later leaned into one of their defenders in the box for a header and the defender gets a free kick, and gets out of a tricky situation. That's where people get annoyed with the inconsistent reffing. How is leaning into an attacker in on goal, to take him out, not a free kick but the other way round is?

All being said, looking back at McGree's chance, he should've buried that.

I haven't seen a conclusive replay but I thought that the keeper got something on McGree's effort? The ball seemed to loop upwards allowing the defender to get back and clear? Just a feint touch but enough to take a little pace off.

Yeah keeper got a touch but he still should have buried it.
 
Is it still the case that if a penalty had been given Dijksteel would have just received a yellow and not a red? The 'triple punishment' law.
In hindsight that would have been the better outcome.
IMO not applicable in Dijksteel's case.

I looked it up after the game yesterday & the Law says:
Where a player commits an offence against an opponent within their own penalty area which denies an opponent an obvious goal-scoring opportunity and the referee awards a penalty kick, the offender is cautioned if the offence was an attempt to play the ball; in all other circumstances (e.g. holding, pulling,
pushing, no possibility to play the ball etc.) the offending player must be sent off.
